30-day notice. Billing continues for 30 days from the day we receive this request. The notice period is not waived or prorated.
Effective date. Your membership ends 30 days from the day we receive this form. We confirm the date in writing.
3-day buyer's right (Washington law). If your membership started within the last 3 business days, choose the matching option below for a full refund.
Disability, death, or move. Washington law allows immediate cancellation with prorated refund if you become totally disabled, if a member has died, or if you move more than 25 miles from the gym. Choose the matching option and we will request the required documentation.
